MEDICAL
Your options under a comprehensive health
insurance plan include a Preferred Provider Option (PPO) underwritten
by UniCare Life and Health Insurance Company. This arrangement eliminates
gatekeepers and primary care physicians as well as any requirement for
referrals required under EPO and HMO arrangements, providing our employees
with unrestricted, self-directed access to the healthcare they need through
providers of their choice. There is a per office visit co-payment of $25 for
in network providers and $35 for in network specialists. Annual deductibles
range from zero under the Preferred Option Plan (POP) to $100 in and $500 out
of network under the High Option Plan (HOP). Prescription drug coverage is
provided under both plans with co-payments of $10/$15/$20. There is also
a per-hospitalization deductible of $100 under the POP and $250 under the HOP.
The primary difference between these two plan options is that the POP is
designed for employees electing to exclusively use in-network providers and
the HOP is designed to provide an employee with the flexibility to utilize
both in and out of network providers.
